This paper describes the main stages of the scientific career of Gaetano Pieraccini, on the occasion of the reorganisation of the Pieraccini collection in the National Library of Florence. In many ways Pieraccini's life parallels the evolution of Occupational Medicine in the first half of the XXth century. Although it begin with the foundation of the "Clinica del Lavoro" in Milan by Luigi Devoto, Occupational Medicine was compromised during the Fascist regime but took on a new life in the post-war years, vicissitudes which were reflected in the large collection of Pieraccini's letters in the National Library.
